Hi doctor,

I had a lipid profile test last month, and it showed the following values such as cholesterol -191 mg/dL, triglycerides - 533 mg/dL, HDL cholesterol - 29 mg/dL, LDL cholesterol - 55 mg/dL, VLDL cholesterol - 106 mg/dL and Non-HDL cholesterol - 162 mg/dL. I never did a lipid profile test earlier. The thyroid and blood sugar tests are normal. I have had a hormonal imbalance and PCOD problems since last year. I am currently taking tablet Normoz-DS, suggested by a gynecologist. In addition, I have taken tablet Rosuvas 20 mg one tablet at night for the last 25 days for triglycerides. The doctor suggested the above medicine for 30 days. These are some queries that I need your help with. What should be the subsequent treatment after 30 days? Which medicine will you suggest in this case? Please mention this in detail. When should I do the next lipid profile test? How serious is my triglyceride level since I also have PCOD? Which foods can reduce triglyceride levels? Kindly give your opinion.

You have to modify your lifestyle, or it will return to its previous state after the medication is stopped. So important thing is you do lifestyle modifications like reducing intake of oily fried food, increasing intake of salads and green vegetables, avoid egg yellow and meat (if non-vegetarian), having a light dinner, have toned milk, avoid ghee, avoid additional oil in the food, do regular exercises like brisk walking for at least 30 minutes a day. Also, you should achieve some more weight loss. Now you should recheck your cholesterol level after 30 days of medications. If it is normal, then a dose of tablet Rosuvas (Rosuvastatin) should be reduced to 10 mg for another month (consult with a specialist doctor, talk with him or her and take medicines with their consent). You will again need to retest after one month of dose reduction to confirm that it is not rising again. If testing shows reasonable control and you can achieve a good healthy lifestyle, we can try stopping it later. So, it would help if you focused on healthy lifestyle modifications and calorie restriction and should undergo a repeat lipid profile to see the response. Thank you so much for the detailed reply. It has been a month of taking tablet Rosuvas 20 mg. I did my lipid profile test yesterday, and it showed the following values. I have a cholesterol range of 105 mg/dL, triglycerides of 233 mg/dL, HDL cholesterol of 30 mg/dL, LDL cholesterol of 28 mg/dL, VLDL cholesterol of 46 mg/dL, and non-HDL cholesterol of 75 mg/dL. I have attached the report for your reference. These are some queries that I have to know about it. What is my triglyceride level in the latest report? Which medicine will you suggest after this report? When should I do my next lipid profile test?

From the reports attached (attachment removed to protect the patient's identity), your triglycerides have come down to 233 mg/dL from 533 mg/dL, but LDL (low density lipoprotein) has reduced (not a major issue). I suggest (consult with a specialist doctor, talk with him or her and take medicines with their consent) Fenofibrate 145 mg once daily at bedtime after dinner. This medicine is specifically given for triglycerides. Take this for six weeks and repeat the lipid profile after six weeks to see whether it is under control or not. As I mentioned earlier, dietary modifications and regular activities are must be followed. All the best.

I have taken tablet Fenolip (Fenofibrate) 145 mg once daily for the last six weeks. I did a lipid profile test yesterday and it showed the following values, cholesterol range of 167 mg/dL, triglycerides of 286 mg/dL, HDL cholesterol of 38 mg/dL, LDL cholesterol of 71 mg/dL, VLDL cholesterol of 57 mg/dL, and non-HDL cholesterol of 129 mg/dL. From the reports attached (attachment removed to protect the patient's identity), triglycerides are still mildly elevated. Tablet Fenofibrate alone is unlikely to control it completely. So I suggest (consult with a specialist doctor, talk with him or her and take medicines with their consent) tablet Rosuvas F (Rosuvastatin) 10/145 mg at bedtime. You should repeat the lipid profile after eight weeks. The likely cause for raised triglycerides is genetic. I guess that you must have made lifestyle changes like avoiding an oily fatty diet and having regular exercise. With genetic cause, one has to be on long-term medications. Once the medications are stopped, triglycerides will start rising. So continue taking medications as mentioned above and make some significant lifestyle changes so as to reduce dependence on medications. All the best.

These are some queries that I have in this regard. Whether the medicine name is tablet Rosuvas F10 or Rosuvas F10/145 mg? I find Rosuvas F10 having Rosuvastatin 10 mg and tablet Fenofibrate 160 mg. I do not think that it is the same medicine that you have prescribed. Please suggest some other brand that is having Rosuvastatin 10 mg and Fenofibrate 145 mg. How long should I continue tablet Rosuvas F10? What is the safe range of triglycerides in women?

I suggest (consult with a specialist doctor, talk with him or her and take medicines with their consent) tablet Lipirose-F10 (Rosuvastatin 10 mg and Fenofibrate 145 mg). You can continue taking it for 6 to 8 weeks and repeat the lipid profile after that. Meanwhile changing diet and lifestyle to a healthy one is important. We would want triglycerides to be less than 200 mg/dL for a good outcome, although triglycerides are not as important as LDL cholesterol. All the best.

I was taking tablet Lipirose-F 10 for the last two months every night. I did a lipid profile test yesterday and it showed the values such as cholesterol of 212 mg/dL and triglycerides of 1104 mg/dL. The report is attached here for your reference. I am taking tablet Diane-35 for the last two months. I have been suggested this medicine due to PCOD and fibroids. Yes, it can be related to tablet Diane 35 (Cyproterone and Ethinyl estradiol). You should discuss with the treating gynecologist to discontinue this medicine. It can cause severe hypertriglyceridemia and increase the risk of pancreatitis. With these, it is better to visit a nearby cardiologist as the level is severely elevated and emergency purposes. You should continue the same medications tablet Lipirose-F10 (Fenofibrate and Rosuvastatin) till the level reaches below 1000 mg/dL and subsequently, medicines need to be modified. However, besides discontinuing tablet Diane 35, the most important thing is the dietary restriction of fats, both good and bad. Restrict fat intake (oils and fried products). Also, reduce sugar intake. Repeat lipid profile after three days of vigorous dietary modifications, and lipid profile should be done in fasting state.

Thank you for the reply. It has been eight days since I consulted. I stopped tablet Diane-35 immediately after your reply. I took tablet Lipirose F10 (Fenofibrate and Rosuvastatin), did lots of exercises, and did dieting in this period. My triglyceride level has increased again as per lipid profile done yesterday. The values are as follows: cholesterol: 208 mg/dL, triglycerides: 652 mg/dL, HDL-cholesterol: 36 mg/dL, LDL-cholesterol: 41 mg/dL, VLDL-cholesterol: 130 mg/dL, Non-HDL cholesterol: 172 mg/dL. I did haemoglobin test one month back and its value was 10.70 mg/dL. I am feeling tingling sensation in the hand and legs with weakness in body sometimes. The low haemoglobin is due to heavy periods that I have because of multiple fibroids. I am currently taking ayurvedic medicines since last one month. These medicines were prescribed for fibroids problem. I am also taking tablet Folic acid 123 once daily. In the last few days I had chest pain few times. At the time of chest pain there was gastric pain in the stomach. I took tablet Pan-d to manage gastric and chest pain. I never had this issue earlier. These are some queries that I have. Regarding the cause for elevated triglycerides, it may be due to genetic component or PCOD and some contribution might be from the current medications. You should consult an ayurvedic specialist to get more information regarding the side effects of these medicines. However, since these are also hormonal preparations, they can have this side effect as well. If you have adequately changed your lifestyle then you will need to be on long term medications (till disease activity goes down). As of now, as tablet Lipirose-F 10 worked well in you, you should have it, and continue it for atleast few months. You again need to repeat lipid profile after six weeks. You also need to have aggressive dietary modifications and regular physical activities. For low hemoglobin, you can have tablet Orofer XT (Ferrous ascorbate and Folic acid) twice daily after some food. This also needs to be continued on long term. Hemoglobin is mildly low. And you will also need some treatment to control blood loss, so consult a gynaecologist as well. You should get ECG (electrocardiogram) to ascertain the cause to be gastric rather than cardiac.

The genetic component is probably playing more roles in the elevation of triglycerides. Tablet Lipirose-F 10 is working well, so it can be continued. Once more you can give a trial of a healthy lifestyle along with diet modifications. Stop tablet Lipirose-F 10 for four weeks and repeat the lipid profile. If triglycerides goes back to the previous range then that confirms the cause is genetic and it will require life long medications, which can be tablet Lipirose-F 10, later can be changed to tablet Fenofibrate 160 mg (consult with a specialist doctor, talk with him or her and take medicines with their consent). You should continue to take all dietary precautions and continue exercises. However, these medicines need to be stopped before pregnancy.
