Patient's Query
Hello doctor,
I am writing out of concern regarding a potential HIV exposure. I had a sexual encounter involving unprotected oral and vaginal sex. Since then, I have taken fourth-generation HIV tests at day 16, day 23, and day 53 post-exposure, all of which returned negative.
At day 53, however, my wife developed significantly swollen and tender lymph nodes, which has deeply worried me about the possibility of a false negative result on my part. I understand that fourth-generation tests are considered conclusive at 28 days and beyond. Is that still the consensus?
Additionally, I have read that high doses of biotin can interfere with certain lab tests. I have been taking a multivitamin containing 300 mcg of biotin daily. Most sources suggest that only doses over 5,000 mcg pose a risk of interference, but I remain anxious about any potential impact on my test results.
This situation has caused me a great deal of anxiety, and I am extremely distressed by the idea that I might have unknowingly transmitted something to my wife.
My main questions are:
Kindly assist.
Thank you.
Hello,
Welcome to icliniq.com.
Your 53rd day negative test can be taken as conclusive, as six weeks is a good period for the fourth generation ELISA (Enzyme Linked Immunosorbent Assay). Biotin does not affect test results. But anxiety and HIV phobia after exposure are very common. Try not to research online much, as you will mentally disturb yourself with false information. Swollen lymph nodes in your wife could be due to some other reason or infection.
Please let me know if you have any other doubts.
Thank you.
Patient's Query
Thank you, doctor.
Have you read the studies on Biotin? I am sorry for the doubtful response. Her lymph nodes are painful, and no other symptoms. I am just panicking silently.
Kindly assist.
Thank you.
Hello,
Welcome back to icliniq.com.
Yes, I have read the reports. These reports and warnings are still in the investigative stage, and to date, no clear-cut guidelines are out regarding the definitive role of Biotin in HIV (Human Immunodeficiency Virus) testing. Moreover, as you said, the dose of Biotin you were taking is quite less than what is assumed to interfere with your test results. Thirdly, you have tested thrice, and all of these are negative. These factors highly incline your case towards a negative HIV status, and there is no need to worry. Your wife may need a physician's examination to determine any other cause of tender nodes.
I hope I have answered your query.
Please let me know if I can assist you further.
Thank you.
