Can Omalizumab help safely manage my chronic hives?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello Doctor,

I am 46 years old and have had chronic hives for years. Should a 46-year-old who takes daily antihistamines ask about Xolair if the tablets are no longer working effectively?

I take multiple pills each day and still experience itching and swelling. My sleep is poor, and I am constantly anxious about when the next flare-up will occur. The antihistamines make me feel drowsy and mentally foggy.

I have heard about Xolair, but I worry that it may be too aggressive a treatment or unsafe for long-term use. Is it a reasonable option at my age?

Living with this condition feels unbearable and is affecting both my work and personal relationships. I do not want to overreact, but I also cannot continue suffering every day. I would appreciate guidance on the next steps.

Kindly advise.

Hello

Welcome to icliniq.com.

I read your query and understand your concern.

The condition being described is exactly the situation in which moving beyond daily antihistamines is not considered overreacting but is actually the standard next step in treatment.

Chronic hives that persist for years despite the use of multiple antihistamines are often classified as treatment-resistant chronic spontaneous urticaria (CSU).

In such cases, Omalizumab is a well-established and widely used treatment option rather than something extreme or experimental. It is not a traditional immunosuppressive medication, and it has been studied extensively and used for many years in adults with chronic spontaneous urticaria, including individuals in their 40s, 50s, and older age groups.

Many patients who experience persistent itching, swelling, fatigue, and dependence on multiple antihistamines achieve a significant reduction in symptoms, and some experience near-complete control of their condition after starting Omalizumab treatment.

Concerns about long-term safety are understandable. However, Omalizumab has demonstrated a favorable safety profile when prescribed and monitored appropriately. For many individuals, the improvement in sleep, daily functioning, and overall quality of life outweighs the burden of continuing with poorly controlled symptoms.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
