Hello doctor,

I am 27 years old with mild ET and thalassemia. I am only taking the tablet Clopidogrel as I am allergic to Aspirin. My doctor said I am at low risk and do not require any other medications. I want to lower my platelet count. I have Hydroxyurea with me now, but I want to know the proper way to administer it. I was prescribed Hydrea before but stopped it after three days because I was afraid of the side effects. I want to lower my platelets because I feel tired. My doctor told me to take the Hydrea tablet once daily every morning. Will that be sufficient?

I suggest you take 800 mg of the prescribed tablet daily and Aspirin 75 to 150 mg. Consult a specialist doctor, discuss with them, and take medications with their consent.

My platelet count is already high. I have taken Hydrea tablet twice a day but stopped it today as my platelet got higher. My platelet count was low last week. What shall I do? There seemed to be something that made my platelets go higher even when I took the Hydrea tablet. Could stress be responsible for it? I have been quite stressed lately. I have attached my CBC reports.

I have reviewed the attached reports (attachments removed to protect the patient's identity). Hydroxyurea therapy is based on bone marrow suppression. I expect it to start responding after two to three weeks. Hydroxyurea therapy is usually advised when the platelet count is more than ten lakhs. I suggest you start taking it, but it will take time to respond. Stress usually does not cause an increase in platelets. However, mild bleeding anywhere or menstruation may trigger the condition. For immediate reduction, plateletpheresis is available.
