Hi doctor,

My blood pressure is 150/90 mmHg, and I am 18 years old. There is pain in the neck and at the top of the head.

Currently, I am taking Arbitel 250.

I noticed that you have grade 1 obesity (attachment removed to protect patient identity). You will need to give more history, for example, do you have a family history of hypertension. The neck swelling needs to be evaluated to rule out a thyrotoxicosis.

The following investigations are recommended: urinalysis, serum electrolyte urea and creatinine, fasting lipid profile, fasting blood glucose, chest radiograph, urine VMA (vanillylmandelic acid), and a neck scan.

It is important to rule out substance use disorder as a differential diagnosis. The probable diagnosis appears to be an endocrine cause of hypertension.

You should see a doctor to prescribe appropriate antihypertensive medication. Preventive measures include reducing salt intake and adopting lifestyle modifications such as weight loss, regular exercise, reducing alcohol consumption, and quitting smoking.
