A raised scar that has been present since a long time is most likely either a hypertrophic scar or a keloid. Hypertrophic scars stay within the boundary of the original injury, while keloids grow beyond it and may continue enlarging over time. Since this scar is long-standing, creams alone usually do not flatten it significantly.

Silicone gel or silicone sheets can sometimes soften and slightly flatten the scar if used consistently for several months, but more effective treatments include intralesional corticosteroid injections (such as Triamcinolone), which help reduce thickness and itching.

In resistant cases, dermatologists may combine steroid injections with 5-Fluorouracil, laser therapy, cryotherapy, or pressure therapy, depending on the scar’s size and location.

Surgical removal is possible but must be combined with other treatments to reduce the risk of recurrence, especially for keloids. I recommend an in-person dermatology evaluation to determine the exact type of raised scar and tailor the best treatment plan for you.
