Hello doctor,
I have a constant problem with my head spinning and vomiting. Recently, this has started (during the last three months). Suddenly, it feels that my head is spinning and then after a few minutes I feel to throw up. I almost require a support during my head spinning. Afterward, (after I lay down in bed and within a few minutes), the head-spinning is gone and I just feel a slight headache. It occurs almost twice a month. Often it is accompanied by stomach pain but not always. However, I do not feel any chest pain or any other problems.

I have seen all your reports (attachment removed to protect patient identity). ECG and 2D echo which are showing no heart disease and cardiac output are also good. Now, we will come to your complaint. Spinning followed by vomiting, the first reason may be due to orthostatic hypotension when you suddenly get up, you feel spinning due to reduced blood pressure. It shows autonomic failures. The second cause can be vestibulocochlear neuritis that is in the ear, they maintain our body balance, but due to swelling, they cannot maintain and you feel spinning sensation and vomiting. For this, you can take tablet Vertin 16 mg once a day and once visit ENT doctor nearby and check your BP at lying and sitting position.

For this, you have to do MRI brain to rule out intracranial pathology and we can also rule out the vestibulocochlear disease. The brain clot might be there but it does not cause spinning as blood pressure fluctuations. You go for MRI. Do routine tests like CBC, KFT, serum electrolytes and contact me with reports.
