Hi doctor,

I am suspected of having mild dysautonomia after my COVID infection. I have increased heart rate and blood pooling in the lower limb on standing. I am taking tablet Escitalopram currently. A recent blood test showed an MCV of 101.8 fL and a red cell count of 4.41 million/mm³. Should I be concerned about these metrics?

Following is my opinion for your asked question.

The only significant finding in the attached report is a high B12 due to vitamin B supplement intake (attachments removed to protect the patient’s identity). It is a water-soluble vitamin, and it will be excreted in the urine in few days.

1. An increase in vitamin B12 level could be due to liver disease, but your liver profile report is normal. So the liver is not the cause.

2. Your total WBC (white blood cell) count and other CBC (complete blood count) parameters are also normal and not suggesting any other cause of elevated B12 like blood cancer. So blood leukemia is also not the cause.

3. Excess non-vegetarian food also can cause elevated vitamin B12.

In short, the most probable cause seems in your case for high vitamin B12 is supplement intake. Kindly give the history of such vitamin supplements intake if present.

Do not worry about elevated vitamin B12 levels. It is due to vitamin B12 supplement intake. As it is a water-soluble vitamin, it will be excreted in the urine. You can complete the course on vitamin B12. Once you stop taking the supplement, the level of B12 will fall within the range.
