Hi doctor,

I want to conceive naturally but my periods are irregular. I had undergone ultrasound and blood tests. My antral follicle count is zero. My anti-mullerian hormone (AMH) levels and follicle-stimulating hormone (FSH) levels are also low. My laboratory reports are attached. I have been taking tablet Eltroxin 25 mcg, tablet Regestrone 5 mg, and tablet B Long F as suggested by my doctor. Please help me.

I understand your concern.

Let me share a few facts on pregnancy. For every lady, the maximum number of eggs is present in the fifth month inside the mother's womb, which is around seven million. Then at birth, it reduces to one million, as the not-so-healthier eggs will get destroyed on their own. When you reach adolescence, it is around four to five lakhs. You ovulate about 400 to 450 times in a lifetime. In this race, always the best, healthiest egg with the maximum number of follicle-stimulating hormone (FSH) receptors will be chosen and sent forwards for ovulation every month. So as time passes, the number of healthy eggs left after every period will be less than in the previous cycle. There is no medicine or treatment to bring more healthy eggs. As you cross the age of 35 years the number of healthiest eggs is low and after 40 years, the eggs are too low for getting pregnant. There can be exceptions. The oldest lady to have a baby naturally was 45 years. So there is no absolute age to have a baby. In this context, the anti-mullerian hormone (AMH) forms a fence-like partition around the ovary to stop all follicles from coming out at a time. So at a young age, this is very high so that only the eggs on top will be able to jump over the fence. As your age advances, the number of eggs reduces, and the level of AMH comes down so that eggs in the lower range can be released. The follicle-stimulating hormone (FSH) is a hormone from the brain, which stimulates the eggs to be released from the ovary. At a younger age when there are lots of eggs, a small dose of FSH is enough. As age advances, the eggs are reduced and a higher dose of FSH is needed to release eggs. So there is a controversy in your results. As age advances, FSH increases and the AMH reduces. Age is a risk in your case, but this cannot be absolute. According to your reports, the size of the ovary is very small and suggestive of low follicle count. But there can be chances of pregnancy even with low ovary size. Thyroid-stimulating hormone (TSH) and luteinizing hormone (LH) are not very important. AMH is very low, which suggests poor follicle count. FSH value is altered by the hormone treatment. A good flow of periods after withdrawing hormones indicates abnormal hormone levels. Considering your age, scan, history, and blood results, you have better chances of pregnancy only after treatment. Since you do not have any other problem, you may get pregnant by natural methods. But chances are very low. As there is less number of eggs, sperms cannot meet eggs. The eggs now left will be less healthy, so the embryos formed will be less healthy. Hence, even if you get pregnant, chances of embryo anomalies and abortions will be high compared to a younger age. If you ask me, the best and most effective method to get pregnant would be in-vitro fertilization (IVF) with donor eggs. This can be tried at any age.

Your main problem is eggs are not healthy and are less in number. You can get a healthy egg from a donor in the same way as you receive blood for anemia. It can be taken from your friend, your husband's relative, or professional donors. They have to be less than 32 years, it does not matter if they are sterilized or have a baby. The donor will be given medication and multiple eggs will be stimulated. These are removed and frozen in the laboratory. Your husband's sperm will be collected on the same day and injected into the frozen egg. The embryo is ready and stored in your name. Your uterus is prepared to have a good thickness of inner lining. The embryo is implanted into your uterus. This can be kept confidential, only the doctor, you, and your husband will be informed. The remaining embryos can be frozen in your name and can be kept for a later date. You can have a baby as long as embryos are frozen in your name. I know every lady wants to have their baby in a natural method, but such problems as low egg count have to be accepted.
