Hello doctor,
I have had redness on the outer groin for a month or so. Now it is red and raw in the groin crease and painful because of the friction. Also, a couple of new growths I have never noticed before. Are those skin tags, warts, or moles?

According to the clinical image attached (attachment removed to protect patient's identity), it appears to be intertrigo (superficial fungal infection due to chronic friction, sweating, occlusive tight wear). Apply Luliconazole cream twice daily for three weeks or Miconazole 2 % cream (Micogel) twice daily for three weeks. Apply Adzorb (Clotrimazole and Lidocaine) dusting powder immediately after bath for two months. Wash the area with Ketoconazole soap or Candid (Clotrimazole 1 %) soap, or Dersil AF (Ketoconazole 2 %) body wash for two months. Keep the area clean and dry. Avoid moisture or heat, or activities that cause sweating. Avoid possible triggers. For skin tags, thermal cautery is the treatment of choice. Are you sexually active, or have you had unprotected sex in the last three months? Any history of stress? Have you done excess scratching or rubbing on the lesions? Since when did the rash appear? Any history of injury in the past? Answer the questions for details. Thank you.

I am sexually active but have not had unprotected sex since last November. No history of past injury. I have had irritation for the last two months when the weather gets warm.

From the symptoms you have described, I suggest that it is scrotal dermatitis. Apply Aveeno moisturizing lotion (Dimethicone 1.2 %) at night for 21 days. Apply 1 % Hydrocortisone gel in the evening or Desonide lotion in the daytime for seven days. Avoid scratching and rubbing lesions and maintain proper hygiene. Tablet Loratidine 10 mg once a day after breakfast for ten days (if irritation persists). Keep a check on blood sugar levels. Stay in a well-ventilated and cool environment. Avoid occlusion, friction, injury, trauma, or tension at the site. Follow up for any doubts. History of surgery in the past? History of testicular torsion? Answer the questions for details.
