Why do I have extreme pain and difficulty in passing stools?

Hello doctor,

I suffer from stomach pain, throwing up, bright red to dark black blood in stool, and a constant urge to go to the toilet. However, I cannot finish going to the toilet, and the stomach pain is so bad it is hard to do anything else. I have had these symptoms for two and a half years. I went to two doctors and was dismissed. Now a new gastroenterologist said it is IBS-D. I do not have diarrhea whatsoever and have been waiting to get a prescription for Xifaxan for over three weeks. I tried to contact his office the last two days because I have been in so much stomach pain I have been moaning in pain. I also went to ER four months ago and was diagnosed with epiploic appendagitis. I do have a family history of ulcerative colitis. I am a 22-year-old female, 5'5 height, and weigh 190 lbs. I am taking Effexor XR 150 mg, Adderall XR 25 mg, and birth control (Sprintec). I tried Dicyclomine and inflammatories, but it has not helped at all. I have high white blood cell count and am highly anemic.

I need help as I feel like no one is listening to me. Please help.

I can understand the reason why you are upset. But the information you have provided is somewhat insufficient to reach a conclusive diagnosis.

First of all, irritable bowel syndrome (IBS) does not cause blood in stools. So it must be another reason for it. Did you have colonoscopy or gastroscopy done? I want you to tell me a detailed history like the duration of the symptoms and the prescription you have received for your abdominal tissues? For example, what is the course of Effexor XR (Venlafaxine) and Adderall XR (Dextroamphetamine-Amphetamine) use? What is the quantity of blood in stools? What were the results of any blood and stool testing that has been performed? Did you see a gastroenterologist while you had your symptoms?
