Hello,

My mother is diabetic and hypertensive. She had some joint pain about two months back, for which she took painkillers for about 20 days and took Trinerve capsules for vitamin B 12 for about a month. Her HBA1c was 7.6 %, so her sugar medicines were also changed. Earlier she was taking Glycomet GP3 850 and Voglibose 0.3 twice a day. Now the doctor has changed it to Glycomet Trio 2 SR and introduced Gliptagreat 50 twice a day. These new sugar medicines she has been taking for the past three weeks. The past week, she has had some itching in her feet and hands without rashes. The doctor suggested Allegra 120. After one dose only, she is better, but her CBC, LFT, and B12 tests were also done. Please see her reports liver enzymes are mildly high. Could it be due to the painkillers she had taken? Could it be due to the new medicine Gliptagreat? Is itching related to any of these or high B 12 because of supplements? I will also share the report of tests done two months back. I hope there is nothing to worry about. She also takes Telmisat AM, Alprazolam 0.5, Nexito 10 mg, Glycomet GP3, and Ecosprin 75 mg. Please help.

I read your query and understand your concern. I saw your mother's reports (attachments removed to protect the patient's identity), and I believe that the analgesic use must be stopped immediately as it can be one of the possible causes of raised liver enzymes. Secondly, if vitamin B12 injection was used, it can also be a reason for the raised liver enzymes. So she must stop B12 supplements totally for now. I suggest she must go to her doctor again and assess her liver by ultrasound. This is definitely a cause for concern. She has the itchiness of the foot due to liver enzyme derangement. Kindly visit a hematologist as soon as possible to assess her liver. Thank you.

Hi,

We have stopped painkillers already a month back and b12 supplement now. Should we wait for a month to repeat the test? Is Gliptagreat causing any problem? She also suffers from anxiety, so rushing her might increase it. Itching is better after taking Allegra. Thank you.

I understand your concern. Gliptagreat is an excellent drug with no effects on the liver. You can recheck after a week. If you feel she has any problems, then you can go to the doctor at that time. Thank you.
