Hi doctor,

I am a 29-year-old male. For the past four months, I am having immense pain in the testicle area, and one of the testicles is swollen. When I keep an ice pack the pain is reduced for less than two hours.

I have gone through your query, I would like to know whether you had any injury to the testis prior to the start of the symptoms. Do you have any other symptoms like a burning sensation during urination or poor urinary stream? At your age, the most important thing is to make sure that the testis is not twisted on itself, which can cause pain and swelling and needs immediate treatment. This has to be decided by a clinical examination by a urologist, so I would suggest you visit your local urologist at the earliest. It can also be due to infection or a fluid accumulation (hydrocele), but these can be treated easily and are not emergencies.

Thank you for the reply,

Currently, this is the physical appearance of the swollen testicle. Is there anything that you can predict as to what the condition of the testicles would be? Are there any chances of damage to the testicle? Is there any preliminary or emergency first aid that I should follow till I get the insurance support for a detailed check-up so as to keep the condition under control? I have not had any injury before in that area and also there is no issue with urination.

It is not possible to confirm without a physical exam. If the testis is painful and enlarged, it is most likely to be either an infection or a twisted testis. Keeping the testis supported will relieve some pain in infection, but if it is twisted this will not help, it will need urgent treatment only.
