I have a rash on my inner thigh of both legs. It is not too itchy, but it itches intermittently that too mild. It has been few weeks since I got the rash. I applied Candid total cream, but it did not help. What should I do? Please guide me.

You may be suffering from a fungal infection, which commonly affects the inner thighs, groin, and buttocks area. It can be treated by a course of anti-fungal treatment, which includes tablets along with creams. Do not use soap over that area. Mistreatment with the steroid containing creams like Fourderm (Chlorhexidine Gluconate, Clobetasol, Miconazole, and Neomycin), Panderm (Terbinafine, Clobetasol, Ofloxacin and Ornidazole), Quadriderm (Beclometasone, Neomycin, and Clotrimazole), Dermi 5 (Clobetasol, Gentamicin, Clotrimazole, Clioquinol (Iodochlorhydroxyquin) and Tolnaftate), Betnovate (Betamethasone), Dermikem-OC (Clobetasol, Neomycin, and Miconazole), and Candid-B (Beclometasone and Clotrimazole), etc. lead to repeated cases of recurrence and also side effects due to application. Do not use steroid-containing combination creams. Do not delay treatment as it may spread further on your body and other people living in the household. Please send me the picture for further help.

Thank you. I assumed it to be jock itch. And I do not have it on the skin folds. I have a total of four patches, but it is not supposed to be very itchy. It is there for a month, and it has not spread. It is like many raised lesion which are red, it is not completely flat, and most of them are not even ring-shaped. Kindly help me

Jock itch or fungal infection are both the same, and itching might not be that severe in some mild cases. And did you apply any creams? Please send me a picture so that I can guide you appropriately.

Candid total (Clobetasol, Miconazole and Neomycin) which you have used (attachments removed to protect the patients identity) is steroid combination cream which can change the presentation and can be responsible for fungal resistance. I hope this was helpful.

I have seen the pictures attached (attachments removed to protect the patient's identity). It is a fungal infection. I will suggest you the following medications. Take Tablet Sebifin (Terbinafine) 250 mg, daily night one tablet for 15 days, Tablet Levosiz (Levocetirizine) 5 mg, one daily night for 15 days and use Onabet cream (Sertaconazole), for local application twice daily. Course may include an extension for one to one and a half months in some individuals. I hope this was helpful.

Thank you. Should I consult after 15 days? I started taking Montek-LC for two days. Should I continue that or start Levosiz instead? what precautions should I take to prevent the spread to other parts and other family members? Please guide me.

Montek-LC (Levocetirizine and Montelukast) can be continued. Consult after 15 days, and I will let you know whether you need to continue oral medication or not. As you have used steroid creams in the past, you might have a little burning sensation after applying Onabet, so apply little amounts initially. It would help if you take some precautions for the complete cure. Avoid sharing towels and soaps. Avoid contact with pets. Check for any other family member with a fungal infection. Then he or she should take treatment. Apply prescribed cream or ointment regularly for two months. Apply not only on the affected area but also you should apply on the surrounding normal area. Take a shower twice daily. Dry your innerwear under the sun. Wear loose cotton clothes and avoid tight jeans. I hope this was helpful.
