Hi doctor,

I am seven months pregnant and I have type 4 chondromalacia patella and ACL grade 1 sprain. My MRI report shows small focal articular cartilage erosions in the medial femoral condyle of the knee and mild left knee joint effusion. I am doing strengthening exercises but I have a lot of pain in the posterior part of my knee joint while standing. The doctor suggested me not to take any painkillers till delivery. What to do in this situation? Do I have to go for surgery? I am very worried as I am unable to sleep also. Is it a major issue and does it relate to arthritis?

No, you will not need surgery anytime soon, especially during your pregnancy or post-pregnancy period. Type 4 chondromalacia can be easily conserved. We can definitely help you with your problems especially considering your pregnancy. Most of the painkillers and medications are to be avoided during pregnancy. However, if you have unbearable pain then you can take the tablet Paracetamol 650 mg for controlling pain, which is also safe in pregnancy.

I would also recommend you to do local therapy in the form of hot and cold fermentation three times daily. Apply some local Diclofenac gel or pain-relieving cream on your knee. Do some exercises for knee strengthening and apply a knee cap or a crepe bandage. Other safe medications during pregnancy, for improving knee cartilage condition will be to take the tablet Calcium 500 mg once daily for one month, and the capsule Vitamin D 60000 units once a week for four weeks.

Out of curiosity, I would like to know if I will need cartilage surgery in the future after delivery. Also, are type 4 and grade 4 chondromalacia the same? I am quite concerned and wondering if it can be resolved after pregnancy through non-surgical methods and medication.

After your pregnancy, it is not mandatory to undergo cartilage surgery. Sometimes, chondromalacia can improve with medications and exercises. If you do not experience pain and your condition improves, you can avoid surgery or at least postpone it until you are severely symptomatic. Whereas, chondromalacia is graded based on the severity of cartilage involvement in the compartments of the knee. Type 4 is a severe grade of chondromalacia, but it may not always require surgical treatment. The choice of treatment modality depends on your symptoms, the impact on your daily activities, and your surgeon's preference for treating you.

Is posterior knee pain solely due to chondromalacia? I have been doing stretching and strengthening exercises for the past six months, but there has been no improvement. So, I am wondering if surgery will be necessary. Are you suggesting that type 4 and grade 4 chondromalacia are the same? I have read that type 4 can only be treated through surgery. Is this true, and is the weakness in my knees and legs solely caused by chondromalacia?

If there is no improvement with exercises and medications, you may need surgery after your delivery. Grade 4 is more likely to require surgery compared to milder grades. My suggestion is not to stress about it for now and wait until after delivery. Hopefully, you will continue to improve over the next few months, and if not, surgery may be necessary.
