Hello doctor,

I slipped and fell on my right knee, and there was a popping sound from the knee. I consulted a doctor, and he suggested MRI (magnetic resonance imaging). After looking at the report, he suggested a minor surgery because, as per the report few of the knee ligaments are torn. I have attached the report. Kindly look at it and suggest to me whether the surgery is required.

I read your query and can understand your concern. You have not mentioned one important piece of information. How long back was the injury, and what treatment did you take so far? I reviewed your MRI (magnetic resonance imaging) report (attachments removed to protect the patient's identity). You have a peripheral meniscal tear, a complete tear of the ACL (anterior cruciate ligament), and a sprain of the lateral collateral ligament. While the lateral collateral ligament usually heals, the other two ligaments usually do not heal by themselves and need surgical correction. Both these ligaments are essential for the normal functioning of the knee. Therefore, it is not minor surgery. However, with the available advances in arthroscopic (keyhole surgery) management, the surgery can be performed with very small skin punctures.

Thank you for the reply.

The injury happened one month back, and I have not taken any treatment till now. I ignored it these days, thinking it would heal by itself. But I felt pain whenever I bent my knee or kept it in one position for too long. Then I went for the consultation. My operation is scheduled, and I have a few apprehensions regarding this surgery. I wanted to know whether it would adversely affect my knee in the future. I have heard that the knee joint often becomes very stiff after surgery. Is it correct? Also, tell me how many days of rest are required, what precautions will have to be taken, and for how many days? Kindly help.

I read your query and can understand your concern. What is the surgery planned for you? Is it arthroscopic (keyhole surgery) or open surgery? Open or mini-open knee surgery has higher chances of knee stiffness. But arthroscopic surgery has a very low chance of knee stiffness. However, post-operative knee stiffness also depends on the amount of knee mobility present now. Are you able to bend your knee fully or almost fully now? Regarding rehabilitation after surgery, the best advice will be given by the surgeon who operates on you. However, I suggest the general protocol is to use a knee brace with progressive knee flexion and strengthening exercises for a minimum of six weeks. Jogging is allowed for three months. Sports are permitted at six months.

Thank you for the reply.

The surgery planned is arthroscopic. However, as per my doctor, my range of movement (ROM) is very good. So I suppose it is okay to go ahead. Also, I will be using a knee brace after the surgery. So finally, I should go for the surgery. Thank you for being so concerned, doctor.

I understand the anxiety that you must be going through. Yes, a well-performed arthroscopic reconstruction of the ACL (anterior cruciate ligament) and meniscal repair would be the best for you. Most arthroscopic procedures give good ROM (range of movement) postoperatively with good physiotherapy. Therefore, I firmly believe you should go ahead with the surgery.
