Hello doctor,
I am 27 years old, and my total testosterone level was low, measuring 400 ng/dL. This result was from last week, following a very intense period with insufficient sleep and no exercise. My question is whether I am stuck with this low level and how I can improve it to fall within the normal range.

Male testosterone levels typically range from 250 ng/dL to 850 ng/dL, although reference values may vary between laboratories. Your testosterone level falls within the normal range. If you require further evaluation, additional blood tests may be necessary, including hemoglobin (Hb), total count (TC), differential count (DC), fasting glucose, prolactin, thyroid-stimulating hormone (TSH), and luteinizing hormone (LH).
Low testosterone can also be associated with obesity. Please provide your height and weight in your follow-up. Regular exercise may also help improve testosterone levels.

Thank you for your response.
I do not have any sexual problems, but I am uncertain about my fertility. My height is 6 feet and my weight is 160 pounds. I have not had this test before. I initially thought my sexual disinterest was more than just boredom with my girlfriend. Additionally, I experienced infrequent morning erections. After the breakup and with new partners, I noticed a return of morning erections and a heightened sexual appetite. Recently, however, I have been getting an average of five hours of sleep per night with little to no exercise. My morning erections have decreased from daily to non-existent. I closely monitor my sexual health and feel that a testosterone level of 400 ng/dL seems below average for me. I have attached the test results for your review.
Can my testosterone levels fluctuate between 400 and 600 ng/dL within a month? If so, what kind of improvements can I realistically expect through diet, exercise, and getting eight hours of sleep in a short period?

Your weight is within a healthy range. Your loss of interest might be related to stress or anxiety. I can prescribe medication to help reduce stress and potentially increase your interest. Your testosterone levels are entirely normal. Using external testosterone could suppress your sperm count. Instead of focusing solely on testosterone, you should consider getting a blood test that includes hemoglobin (Hb), total count (TC), differential count (DC), fasting glucose, prolactin, and thyroid-stimulating hormone (TSH) to determine if other hormones might be involved.
While diet and exercise can have an impact, significant changes in testosterone levels within a month are unlikely, as your current levels are normal. Research indicates that testosterone levels can fluctuate throughout the day. For reference, a study found that plasma testosterone levels in healthy males were highest at 4 AM (928 ± 117 ng/dL) and lowest at 8 PM (266 ± 52 ng/dL).
