Hello doctor,

I had a posterior decompression and fusion in t/LIF (2) L4-S1 done eight years ago. Suddenly, this week, I have been experiencing stinging pain in my left buttocks and pain in my left hip joint. Why is that? Kindly help.

I read your query and can understand your concern.

This can be because the nerve might be compressed again. To check that, you will need to take a new MRI (magnetic resonance imaging) scan of the lumbosacral spine with contrast. Until then, I suggest taking the following precautions and medicines:

  1. Avoid lifting heavy weights.

  2. Avoid arching your back; bend your knees to pick up something from the floor.

  3. Avoid sitting for long hours.

  4. Avoid two-wheeler rides.

  5. Improve your posture while sitting.

  6. Try to get up from your chair every twenty minutes, take a quick walk around, and then continue working. Always sit on a chair with back support.

  7. Do not get up suddenly from a sleeping position. Rather, turn to the side and then get up.

  8. Take bed rest if you have severe pain.

  9. Do hot fomentations locally over the affected area at least twice daily.

  10. Apply Oxalgin gel (Diclofenac) for local application twice daily.

  11. Take a tablet of Pregabalin (anti-epileptic) 75 mg once daily for 15 days before food at 7 pm every day.

  12. Tablet Ibuprofen (non-steroidal anti-inflammatory medication or NSAIDs) 400 mg every morning and evening for ten days after food,

  13. Take tablet Rabeprazole (Proton pump inhibitor) 20 MG 1-0-1 X 10 days before food,

  14. Take tablet Shelcal (calcium and vitamin D)500 MG every morning and evening for x 30 days before food,

  15. Tablet vitamin D (dietary supplement)60 units once daily for 10 days, then once a month for 11 months.

  16. Take these tablets only if you are not allergic to them. Do not take these tablets if you have bronchial asthma (Bronchial asthma is a chronic respiratory condition characterized by inflammation and narrowing of the airways, leading to recurrent episodes of wheezing, breathlessness, chest tightness, and coughing).

  17. To alleviate back pain, incorporating physiotherapy exercises, including back extension exercises such as glute strengthening (strengthening butt muscles) and passive straight leg-raising exercises, is crucial. Initially, it is beneficial to perform these exercises under the guidance of a physiotherapist to ensure proper form and technique. Once familiar, they can be continued at home. Commence physiotherapy once the pain subsides, as regular exercise is key to yielding positive outcomes in managing back pain.
