Hello doctor,
I was diagnosed with adenocarcinoma of the lung, stage 4, more than four years ago. Thereafter, regular treatment as per the treatment sequence attached. I am taking Tecentriq 1200, Bevacizumab 800, Genexol PM 300, and Cisplatin 100 mg for six cycles every 21 days. Despite this, the disease is progressing (copy of the last PET scan attached). I have also attached the histopathology report. Please tell, what is the stage 4 NSCLC second-line treatment?

It does happen in stage four of lung cancer (attachment removed to protect the patient's identity). Cancer cells become resistant after 3 to 6 cycles. We need to move to the next line of chemotherapy, which is stage 4 NSCLC second-line treatment. It is the standard protocol. An immune checkpoint inhibitor (immunotherapy) and a taxane-based cytotoxic chemotherapy should not be given together. You can go for an immune checkpoint inhibitor (immunotherapy), an anti-angiogenic targeted therapy, and a platinum-based cytotoxic chemotherapy for four cycles if these agents are not given earlier, and repeat the scan after four cycles. If there is no progression, plan another four cycles.
