What causes burning and cutting pain inside the penis?

Hi doctor,

I have been addicted to the habit of masturbation for 20 years. In the last two years, I used to masturbate daily. When masturbating, I felt something cut off the bundles inside my penis nerves and a lot of burning and pain inside the penis. Initially, I thought it might be a urinary tract infection. Then, I consulted a local urologist, who advised me to go for an ultrasound test, where everything was normal. He suggested that it may be because of nerve irritation and prescribed Gabapentin 100 mg and Carbamazepine 200 mg. Initially, the pain got subsided, but whenever I masturbate and ejaculate sperm, the pain in my penis nerves comes back. I feel somebody pulling and cutting the nerve and burning my penis. Meanwhile, one day, I got some swelling on my penis. The next day it came to normal, and now I am getting that burning and cutting pain inside my penis.

Whenever I masturbate, the pain becomes tripled, worse, horrible, and tortured, lasting for a week. So, I strongly think that it is mainly related to penis nerve damage because of over and excessive masturbation. Will you please help me out? Suppose any tests are required to check whether any internal damage inside the penis has happened or any muscle pain or nerve pain in the penis. Please give a detailed list of those tests that can verify my problem accurately. Please help.

You may have sustained some nerve and blood vessel injury during masturbation. The problem with nerve injuries is that they do not heal completely. Another possibility is due to repeated masturbation. You could have developed a thickening of the tissues in the penis which can compress the nerves. How is your erection now? Has it decreased from before this problem? Is the erect penis straight, or does it curve to one side? Is the pain only during masturbation or even during natural intercourse? Do you have any other urinary complaints? A complete clinical examination to check for any thickening or nerve injuries is needed before any tests or treatment can be started, so I advise you to visit your local urologist again. Then the blood flow to the penis and the internal tissues will have to be checked by an ultrasound scan. Based on these results, any treatment can be planned.

Thank you for your reply.

My penis erection is completely normal; even after this problem, I have good erections. Erection is straight and not curved. Pain will become worse after masturbation. If I do not do masturbation for ten days or so, the pain will gradually decrease, but irritation will be continued, but it is bearable. I do not have any urinary complaints. Please suggest to me what type of tests I should go under to cure my problem completely.
