The redness, swelling, burning, or cutting sensation while retracting the foreskin, and red spots are most consistent with irritant balanitis with superficial skin breakdown, rather than anything serious or permanent. This is treatable and usually heals well if cared for properly.

For now, keep things very simple and gentle. Apply plain petroleum jelly liberally over the affected area two to three times a day to protect the skin and promote healing. In addition, you may apply 1% Hydrocortisone cream as a thin layer once daily at bedtime for only five days; do not extend its use beyond this.

After completing the five days of Hydrocortisone, continue using only petroleum jelly for another 10 to 14 days until the skin fully recovers.

  • Avoid excessive washing, scrubbing, and the use of soaps, antiseptics, or any perfumed products on the affected area, as these can further irritate the sensitive skin and delay healing.

  • Clean the area only with plain lukewarm water, after which it should be gently patted dry without rubbing.

  • Sexual activity and masturbation should be avoided until the skin has completely healed, as friction can worsen the erosion and pain.

  • Keep the foreskin dry and avoid any forceful retraction, which can cause further cracks or injury.

  • Wear loose, breathable cotton underwear to reduce moisture, friction, and irritation and to promote proper healing.

This condition can be distressing, but with gentle care and patience, the skin usually heals completely.
