What does a high mean platelet volume suggest?

Hello doctor,

I just had my routine medical check-up and got my CBC report. My platelet count is 210000 and in range, but my MPV (mean platelet volume) is 12.8.

Is this anything to really worry about?

Please guide.

I understand your concern.

You need not worry about your platelet count and MPV. In your attached report (attachments removed to protect the patient's identity), the total WBC (white blood cells) count is borderline low, and the lymphocyte count is borderline high. So it could be a temporary viral infection. You can repeat your CBC (complete blood count) after a few days to check the count again.

Do not worry. After a few days, repeat the CBC. You can revert once you repeat it.

Thank you for the reply.

Actually, I got a little viral around the first week of two months ago, and I recovered now. But my platelet count is 210000, which is in the range, as its range is 150000 to 400000. But my mean platelet volume is 12.8 FL. Also, I checked my older reports, and it was always between 11 and 12 FL, I mean, on the higher side.

I read on the internet that high MPV and low platelet count are signs of some sort of chronic myelogenous leukemia cancer (CML), and I got worried. I just wanted to clarify that I hope I do not need to go for any testing for CML, so I do not feel any symptoms.

Is it normal to have an MPV of 12.8? Is it really high or marginally high?

Please advise.

Your MPV is marginally high. The borderline elevation does not suggest any blood cancer. In CML, a type of blood cancer, the total WBC count is usually very high, more than one lakh.

Additionally, in chronic myelogenous leukemia (CML), the differential count shows an elevation of basophils. So your attached report (attachment removed to protect patient identity) does not suggest blood cancer. Do not worry.

I think my white blood cells and basophils are on the lower side, and in all reports, they are normal, like 4200 to 5000.

Secondly, what can cause high MPV? Can it be just natural for them to be high? Should I not worry at all about MPV and cancer?

The MPV reflects the size of platelets. It can be high when platelet destruction is present in the blood. Viral infection could also be responsible. You need not worry about MPV. Do not worry about that.

In blood cancer like leukemia, the total count is high and immature granulocytes are present, such as basophils, myelocytes, metamyelocytes, etc., which are not present in your case. So the chance of leukemia is almost nil. You might not need to worry about that.

So, according to the attached report, changes are almost nil or totally nil? Also, is the TLC count low? Does it suggest a viral infection or a bacterial infection? Can it be due to any recent viral infection?

I had a little cough and sneezing, but no fever, and now I am totally fine.

The viral infection seems most likely in your case. You need to repeat the CBC after a few days. Drink more water. The attached report does not suggest leukemia or any blood cancer.

Since my TLC count is low, should I be more worried about getting infected by COVID-19? Is my immunity low, as the TLC count is just 3800? Should I take vitamin C, and I am at home anyway and do not go out at all?

The borderline low level does not suggest weak immunity. COVID-19 can infect an individual with adequate immunity as well. So you are doing it the right way by not going outside the home. If you are going to the hospital area, then wear a mask. Do frequent hand washing with soap and water. Use sanitizer as well.

Vitamin C does not prevent COVID-19 infection. However, it can boost your immunity. You can take it, or you can take more fruit rather than tablets.

Today I checked my temperature, and I have a 98.8 to 99 fever in my mouth, but under my arm, it is 96.5. As such, I do not have any cough, cold, or sore throat. I just had back pain for a few days and was taking Voveron for that. Is a 99-degree Fahrenheit fever in the mouth cause for concern?

Following is my opinion about your question. A body temperature of more than 98.6°F is called a fever. The temperature is usually taken in the axillary area. So keep a thermometer in the axillary region and measure the temperature. If your axillary temperature is more than 98.6°F continuously, then you can take over-the-counter antipyretics and consult a doctor.

In COVID-19 infection, usually, along with fever, weakness, cough, or cold is present. So it does not look like COVID. However, if you have a continuous fever, then it is better to consult your doctor.
