How to identify the cause for skin allergy in a child?

Hi doctor,
My daughter was born six months ago with a weight of six pounds. Her current weight is approximately ten pounds now. From 3rd month of her age, she has got skin problem. We started formula feeding along with breastfeeding at that time. She got skin redness and some sort of problem in her cheek. We have visited pediatrician and he proposed not to worry about that and asked me to use baby milk cream. Nowadays we find that the problem has spread all over her face. It cannot be found in the picture as it is taken just after applying baby cream. Her face has been washed with lukewarm water in the morning and night. And again we are confused with her nutrition. Please provide us good and healthy diet plan that can be taken routinely for her healthy life. Thank you.

I have seen the images (attachment removed to protect patient identity). By what you are telling it is looking like allergic skin disorder. This can be due to anything being applied on her skin, sometimes due to cow's milk allergy, etc. Does your child have fever or any other symptoms along with this? From your answer we might think of alternate diagnosis. What soap or cream or oil do you use for her? Did you change any of the products recently? If so, please use the one you used before. If not try some creams which I suggest. Which milk are you giving her along with breast milk? If cow or buffalo, stop that and try formula.
The following are some changes which I can suggest:
- Start semi-solid foods, which you can prepare at home, like rice with dal. Also banana is easily digestible. Start with these two.
- Start one new item for a week, so that if baby is not tolerating the food you will be able to know.
- Slowly by one year she can eat whatever is prepared at home that is less spicy. You can give any fruit; see that she has some fruit daily.
- Give her potato, tomato, leafy vegetables, carrot, beetroot, etc., with rice or even curd.
- Later at about eight months of age you can start finger millet malt. You can go according with stages of infant food series of available in market.
If the skin problem is still persisting, show to a dermatologist or a skin specialist for further physical evaluation.

Her previous problem has gone with Boroline ultra soft cream. Here is another issue. She is suffering with some sort of sound in chest and back. She is having frequent vomit with cough. There is sudden cough and running nose and it is not too frequent. But, her activities are fine. We have visited our local pediatrician. After her checkup he suggested it as the symptom of primary stage of bronchitis. I have attached the prescription given by him for your reference. He asked me to try nebulizer. Your daughter seems to be suffering from bronchiolitis which is a viral infection of the lungs. It is treated basing on the severity. If no breathing difficulty, we can treat at home with oral medication and nebulization. If severe then we admit in hospital, because child may require oxygen also. From the breathing sound (attachment removed to protect patient identity), it seems that baby is having difficulty in breathing. The medicine given in the prescription is good. Sometimes they might not be enough if problem progresses. Continue the nebulization and visit the doctor if condition does not respond. She may get fever, watch out for it.

She has been given the medication. But, the problem is that she is vomiting when she was given instantly. Local doctor changed the medicine but still she is vomiting. He suggested to use dropper for the same and keep her watch for the next two days with same medication as prescribed. If speed of breathing has increased, please visit the doctor, as she might need admission and the frequent nebulization. If vomiting is more she could get dehydrated. So, could require IV fluids (intravenous). So, please revisit the doctor. The mucus will go in the stools as they swallow it, so loose stool can occur.

Good to hear that she is recovering. Yes, the stools are a bit semi-solid, but not very loose. Some rash is present at the anal region. You can use the following medication for her. The stools could be because of the mucus which she is swallowing and the resultant stomach infection which causes indigestion of the milk. Please reduce her milk consumption. I will suggest her alternate formula for milk temporarily. Once this problem gets over then you can switch back to the normal milk, probably after two weeks. If rash is increasing you can get a stool test done. If it decreases with the following medication then the stool test is not necessary.

It looks like she has an eye infection. It also might be some allergy to something she was exposed at the park. Use Oflox eye drops (Ofloxacin) or Toba eye drops (Tobramycin), four to five times a day for at least three days. Snoring implies some chest symptoms. If her breathing is fine, then you can just use any cough syrup. If she also has breathing difficulty, then better to be seen by doctor as eye and chest symptoms can be related.
