I have white scaling that comes on and off on tongue. What can it be?

Hi doctor,

I am 56 years old and have the problem of white scaling on the side of my tongue and also on top of my tongue. I got tested for fungal growth and it was negative. This problem has been there for one year. I have consulted many doctors regarding the same. They asked me to take extra care of hygiene. Please advise me because I am not getting relief. White scaling comes on and off. I used to brush morning and night. What it could be?

Thanks for choosing this platform. I can understand your concern. As per your query, you have white scaling on the side of your tongue and slightly on top of your tongue. Since you have tested negative for candidiasis or thrush (fungal growth) next possibilities are lichen planus and psoriasis. It can be leukoplakia also but chances are less. Because leukoplakia does not go away on its own. Without examination, it is difficult to tell. So, I suggest you consult a dermatologist or specialist in oral medicine and radiology for a physical evaluation of your lesion.

I have seen your photographs and reports (attachments removed to protect patient identity). Most probably it is lichen planus. It is an immune response of unknown origin. Oral lesions in lichen planus may be asymptomatic or may be painful. I would suggest you to take tablet Hydroxychloroquine (HCQS) 200 mg one in morning and one at night for a week after consultation with a dermatologist, because this drug can have side effect. If there is no response after a week, I would suggest you to have biopsy of the lesion.
