Hello doctor,

I have megaloblastic anemia, but no intrinsic factor deficiency and have been diagnosed for nine years. I have taken CBC and intrinsic factor tests. My current medication is Methylcobalamin. I have an MTHFR gene mutation in both alleles, but I am largely unresponsive to my weekly 1000 MCG injections. My symptoms like burning palms and feet, memory loss, and depression keep getting worse. What should my next step be?

  1. You are having an MTHFR (methylenetetrahydrofolate reductase) gene mutation, so your metabolism of homocysteine can get affected. Homocysteine can be elevated in such a mutation. So check your homocysteine level. Because a high level of it is associated with cardiovascular disease problems.
  2. Check your vitamin B12 level after methylcobalamin injections, your B12 level might have improved.
  3. For your burning palm symptoms, check your thyroid and sugar status as well.
  4. Kindly mention your blood pressure value and smoking history if present.
  5. What was your vitamin B12 value before injection?
