Hello doctor,

I have mild tinnitus and pain at the back of the skull behind the ear. I also have pain in both temples next to the ear. I wanted to ask what causes this as I have on and off pain. Please help.

Is tinnitus constant or intermittent? Is it felt in both ears? Is there any hard of hearing? Do you experience giddiness, especially when you change your posture (turning side to side while asleep)? Was there any trauma to the head any time in the past? Is the pain in your temple region is constant or intermittent? Does it increase with chewing? Do you feel pain when you touch the temple region?

Yes, it pains me when I touch the temple region. It is intermittent, and I do experience giddiness when changing the posture. No trauma to the head in the past, and tinnitus is intermittent only in the left ear. I do not find it hard to hear.

No, I do not feel like vomiting. There is no discharge now or in the past. I use to swim but not in the past month. I soak in a hot bath tub two hours back.

Based on the information, I am thinking of a condition called Meniere's disease. This is a condition wherein there is an abnormal fluid collection in the inner ear. Another condition I am thinking of is bony outgrowth within the inner ear (headache). Both of them may cause symptoms you are experiencing.

It requires a detailed evaluation and treatment accordingly. If there is Meneier's disease, there is no cure but only symptomatic treatment. Despite that, it keeps waxing and waning.

It can be made out on MRI. But I do not suggest it right away. It also requires a few clinical tests, which can be done under the magnifying glass.
