Hello doctor,

I am approximately four weeks pregnant based on the date of conception. I have rheumatoid arthritis and take Methotrexate 10 mg a week, along with Folic acid 10 mg. I stopped taking Methotrexate as soon as I found out about the pregnancy, which was one week ago. So, since conception, I took Methotrexate twice before discovering the pregnancy. Now, I am debating between termination or carrying the pregnancy to term. I am aware that it is strongly advised not to take Methotrexate during pregnancy or even shortly before conception. I am uncertain about what to do, and I am seeking medical advice to make an informed decision. My rheumatologist is undecided and unable to provide further guidance.

You do not need to worry about your exposure to Methotrexate during your four-week pregnancy. As long as it is not an overdose, I do not think it may affect your pregnancy. To be sure, please kindly undergo a fetal biophysical profile or fetal anomaly scan to evaluate for possible fetal abnormalities. You can continue with your Folic acid medication.
