Hi doctor,

My husband has been diagnosed with FLC MGUS (monoclonal gammopathy of undetermined significance) 13 years back. All laboratory test results were completely normal except for his FLC (free light chains) ratio. He is monitored annually. This January his FLC ratio raised from 12 to 21. He had COVID-19 in December. His hematologist suggested that we repeat in July because of COVID. Unfortunately, he got COVID in April's first week also. In May we repeated the test and they went up. Everything else other than the FLC ratio is completely normal.

Is COVID affecting this FLC test? I have all the test results and can submit them for your review.

Please help. Thank you.

I read your query and understand your concern.

It is free kappa that is high. I hope you must have checked serum protein electrophoresis (SPE) for the M spike. If the M spike is negative, no treatment is needed at present. If the M spike is positive, you need treatment. If the M spike is negative, I suggest you repeat CBC (complete blood count), KFT (kidney function tests), serum free light chain ratio, and SPE for the M spike after three months.

COVID can increase the ratio, but there is no proven data. Since the M spike is negative, you should not worry. No treatment is needed at present. But repeat the tests after three months.
