What is the management of MGUS?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

A hematologist has reviewed my reports, and I have been diagnosed with MGUS. My oncologist ordered the laboratories. I want a second opinion.

Hi,

Welcome to icliniq.com.

MGUS (monoclonal gammopathy of undetermined significance) is an indolent type of disease with ups and downs in its course. It is now down, meaning no signs and symptoms and biochemical derangements have been detected. The only abnormality is a minute quantity of kappa-restricted free light chains and slightly high IgG (Immunoglobulin G). It is usually hundreds of folds in treatable patients. Therefore, it does not need to be treated in your case. However, it needs monitoring of renal function tests, calcium, phosphate, blood counts every six months once, and x-rays of skull, backbone, and hip bone, once a year. Meanwhile, with locally available preparations, you can build up your vitamin B12, folic acid, pyridoxine, vitamin D, and iron levels.

Patient's Query

Hi doctor,

Thank you for the advice. Well noted.

You are welcome. I am honored by your words.

Thank you.

Patient's Query

Thank you doctor for the reply,

My doctor has asked me to follow up with laboratory work each month. Is this really necessary? I thought it should be done every six months. I have sent you my reports previously.

Hello,

Welcome back to icliniq.com.

I agree with you. Retest after every six months.

Regards.
