Hello doctor,

I am a 18-year-old and have extremely bad mood swings. I think I might be an attention seeker, I also feel like I have to be upset if someone I know is upset which is ruining my life. I can be happy, then cry the next minute. I also have a problem where I unintentionally say or wish bad things on people I love and care about inside my head, which I know in my heart I do not mean it. This causes worse anxiety than I already have and makes me extremely paranoid to the point where I do not have a healthy sleeping schedule anymore. I have actually not been to bed writing this post and it is morning already. I really just need someone to help me. I feel as if I am going insane. I also sometimes hear people saying 'Ask her, disgusting, or crazy'. This happens a lot when I get the urge to self-harm but I do not anymore so I just ride it out. (The previous therapist said I have anxiety and a germ phobia, I have had trouble with washing my hands a lot in the past) My mom's great grandmother was diagnosed with schizophrenia and a few people in my family have bipolar disorder. Kindly give your suggestions.

I have read your query and understand your concern. There are a few things I want to share with you after reading your symptoms. First of all, you told me that you have a family history of schizophrenia and bipolar disorder. So I would like to inform you that there are chances that you might also get some kind of psychiatric illness. Now as you have mentioned that you had a habit of repeatedly washing your hands in the past, which leads to the probability of obsessive-compulsive disorder or that can be a prodrome also known as the beginning of schizophrenia. I have a few questions for you like, do you get hallucinations any time? Do you feel paranoid that people want to harm you or trouble you by any means? Do you feel that people are talking or discussing about you? It would be great if you answer these questions just to clear the doubts regarding the diagnosis. You already have been diagnosed as having an anxiety disorder with germ phobia. Are you at present taking any treatment? I would suggest keeping track of your thoughts and keeping in touch with a psychiatrist either personally or through the internet. I may suggest some medicines for you only after getting the details from you which I asked for. Meanwhile, for your anxiety symptoms, you can take the tablet Clonazepam (Docusate sodium and Lactose monohydrate) 0.25 mg as and when required. I hope this will help you.
