Patient's Query
Hello doctor,
I am a 58-year-old female recently diagnosed with early Parkinson’s disease. My right hand trembles, and I feel slower in my thinking as well. My ferritin level is low at 11 ng/mL, and my vitamin B12 level is borderline at 210 pg/mL. My questions are:
Could these low levels worsen my symptoms?
Are there differences in how Parkinson’s disease presents in women?
I am worried about the impact on my independence.
Kindly help.
Thank you.
Hello,
Welcome to icliniq.com.
I read your query and understood your concern.
I understand how challenging and overwhelming a new parkinson’s diagnosis can feel, especially with added concerns such as nutritional deficiencies.
I suggest you undergo the following investigations:
Serum homocysteine and methylmalonic acid to assess functional vitamin B12 status.
Complete blood count to check for anemia.
Follow-up ferritin, iron, total iron-binding capacity, and vitamin B12 in eight to 12 weeks.
Improving both iron and vitamin B12 levels may modestly enhance energy, mental clarity, and motor stability. Although this will not cure parkinson’s disease, it can optimize overall neurological health.
I suggest you follow the treatment plan below:
Take iron and vitamin B12 supplementation:
Oral iron supplements, for example, Ferrous sulfate 325 mg once daily with vitamin C for absorption.
Consider vitamin B12 supplementation, oral or injectable, based on follow-up levels.
Repeat ferritin and vitamin B12 testing after eight to 12 weeks of supplementation.
Get a neurology consultation:
Ensure evaluation by a neurologist, preferably a movement disorder specialist.
Medication may not be immediately necessary in the early stages, but regular monitoring is important.
I suggest you can get benefits from the supportive therapy mentioned below:
Physical therapy for motor coordination.
Occupational therapy for daily task adaptation.
Cognitive stimulation, such as puzzles, reading, and memory exercises.
I suggest you follow these instructions:
Ensure a balanced diet rich in iron and vitamin B12, including lean meats, legumes, eggs, and fortified cereals.
Avoid taking calcium and iron at the same time as parkinson’s medications, as this may interfere with absorption.
Practice daily exercise and remain socially active to help maintain independence.
Follow up with a neurologist every three to six months.
Monitor blood levels of ferritin and vitamin B12 during supplementation.
Consider neuropsychological testing if cognitive slowing progresses.
I hope that this answers your query.
Kindly follow up if you have more doubts.
Thank you.
