Patient's Query
Hello doctor,
My father is 70 and has had Parkinson’s for the past five years. He is on Carbidopa-Levodopa and Rasagiline, but recently has developed increased falls and noticeable freezing of gait.
His recent MRI showed mild small vessel ischemic changes, and neuropsych testing showed early signs of cognitive slowing.
Are there any newer therapies or physical therapy approaches to help with his mobility without worsening cognition?
Please help.
Thank you.
Hello,
Welcome to icliniq.com
I understand your concern.
Freezing of gait and increased falls in Parkinson's disease can be challenging, especially when early cognitive slowing is present. Living with these symptoms daily can be physically and emotionally exhausting for both the patient and their caregivers.
Whereas drugs such as Carbidopa-Levodopa and Rasagiline target the motor problems, increasing the number of dopaminergic agents can result in confusion.
In terms of movement, new techniques include cueing in physical therapy (either visual or auditory cues), rhythmic stepping, and even laser walkers and exercise regimens designed for people with Parkinson's.
Staying consistent with these therapies over time can make a meaningful difference in day-to-day movement and confidence.
Assistive devices designed for Parkinson's disease, balance training, and home safety modifications are important to prevent falls. Small changes in the home environment can go a long way in keeping the patient safe and comfortable.
Amplitude-based exercise, Tai Chi, and cycling have provided some benefits to patients without affecting their cognitive function. The presence of caregivers and emotional motivation of patients is also important for their general well-being.
In some cases, adjusting medications to minimize "off" time while avoiding cognitive side effects is necessary, best done under the guidance of a movement disorder specialist.
I hope it helps.
