Hi doctor,

I have a medical condition that I want to discuss with you. I often have a liquid discharge of a few drops throughout the day. This is sometimes associated with arousal and sometimes without, but I often have sexual thoughts also. And I am very disturbed by it. I keep tissue paper in front of my underwear, again wrapped with a piece of a small plastic bag. I have done this for a few years now. Please guide me to get rid of this problem. Sometimes I get discharged after urination (ejaculation without masturbation or arousal). Maybe prostate discharge? I would like to mention that I was addicted to masturbating for a long time (many years) and managed to quit a while ago (one to two years). I was married before and experienced premature ejaculation, and I am sure that the problem is still there. My ex-wife has married again and has a child; she did not get pregnant with me. I have poor and weak erection, premature ejaculation, cannot hold urine, find it very hard to control, and semen is quite thin, these are some of the problems that I face. I have recently been to (Emsella chair) therapy; this is a new technology to strengthen pelvic floor muscle, but I did not find any improvements. I completed the whole course of six sessions, twice a week for three weeks. This Emsella chair was my initiative; a health practitioner did not recommend it.

I have no medical history or permanent medications. However, I often have bad sleeping habits and routines. Lose a lot of hair from the head and the body (no one in my family had this issue). Everyone has and has thick and healthy hair. I can easily get irritated and have mood swings. My heels are badly cracked, especially on the left (dry skin, no eczema). If you require any specific tests, blood tests, urine, sperm, or whatsoever. I can take all the recommended tests. Please help me to get rid of this problem. I have been using tissue paper in my underwear for many years, not to change underwear all the time. Are these drops due to overstimulation of Cowper's gland? Chronic hyperarousal condition? Any prostate-related infection or inflammation? I am looking to get married again in the near future. I look forward to hearing from you soon.

I can understand your concern. What you feel is called pre cum, pre-ejaculatory fluid (PED). It is a normal secretion that comes before you ejaculate so sperm can travel smoothly and get out of the penis. If there is no or reduced PEF (peak expiratory flow), chances fewer chances of pregnancy for a female. You can control the episodes of PED by controlling your erotic feeling. Methods to control erotic emotions are sex education, physical exercises, sleep adjustment, quality of sleep, healthy and balanced diet, programming for leisure time, development of spirituality, and taking a cold bath.

Ejaculation without masturbation or arousal can happen if sexual thoughts without sex or masturbation are not related to the prostate. If you are worried, you need to do an ultrasound pelvis. Masturbation is normal two to three times a week is good, or you need to have sex. And premature ejaculation has no relation to baby forming; you need to check for semen completely to know the cause. The question regarding ejaculation and semen issues, an ultrasound scrotum and pelvis, penis doppler, thyroid function test, blood sugar, serum testosterone free and total, sex hormone-binding globulin, FH (familial hypercholesterolemia), LSH (laparoscopic supracervical hysterectomy), prolactin, PSA (prostate-specific antigen), liver function test and urine complete, semen complete.
