Hello doctor,

I have had a red burning rash on my penis glans for a few years, and it never disappears. The burning comes and goes, but the redness is always present. When I feel some relief, I can urinate, and it makes the redness and burning worse sometimes after a bowel movement. When my penis is erect, the redness and burning are the worse. I was tested twice for STDs, and everything came back negative also. I am not diabetic and have no autoimmune disorders. I have been prescribed tablet Nystatin for two weeks, but I had no improvement. I recently went back to the doctor to explain it all over again. I was prescribed tablet Nystatin and Triamcinolone Acetonide cream for two weeks, and then also there was no improvement in my condition. Please help me. Thank you.

I understand your concern. It looks like a superficial fungal infection only, likely candidal balanoposthitis. If it is recurrent or persistent, get your blood sugar checked once, as it is more commonly seen in people with diabetes. I suggest the following medications. Take Tablet Fluconazole 150 mg weekly for two weeks. Treat the partner with the same if sexually active. Use Clotrimazole cream apply twice a day. Take Tablet Fexofenadine 180 mg once a day for two weeks. Review after two weeks. I hope this helps. Thank you.
