Patient's Query

Hello doctor,

I have noticed the presence of a white creamy substance under the foreskin and on the glans, accompanied by slight itching. This issue has been observed for the past 20 days, and I have observed that the formation of the white substance increases when hygiene is neglected. Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

It appears to be Candidal balanoposthitis, a common condition in men who are diabetic. Typical symptoms include itching, soreness, white patches on the penis head, swelling, irritation, pain, or burning.

I suggest the following:

  1. Ensure sugar levels are monitored, as poor hygiene and improper drying after cleaning can worsen the condition.

  2. I suggest using topical creams containing Clotrimazole or Miconazole, which can be used for 14 days.

  3. Avoid steroid use.

  4. Maintain good personal hygiene, wash the penis head carefully with warm water, avoid scented soaps, and pat dry with a soft towel.

  5. Wear loose underpants for better ventilation.

  6. Infection may occur if sugar levels are uncontrolled, creating a favorable environment for yeast and bacteria.

  7. Identify and treat the underlying cause if recurrent, and consider a urine routine and culture test.

  8. I suggest using Episoft cleanser (purified water, Cetearyl alcohol, 1,3 Butylene glycol, Maltooligosyl glucoside, Hydrogenated starch hydrolysate, Cetostearyl alcohol, Sodium lauroyl sarcosinate, Phenova, Acrylic acid, C10-30 Alkyl acrylate crosspolymer, Triethanolamine) twice a day for two months for genital hygiene. Consult your doctor and take the medicines.

I hope this helps.

Patient's Query

Hi doctor,

Thank you for your reply.

I am experiencing a slight itchy sensation under the foreskin and a mild, non-intense burning sensation. I do not feel pain during sexual intercourse and have no history of high-risk exposure or diabetes. I do not engage in excessive scratching or rubbing. However, I have used coconut oil on my penis, and I am sexually active.

Thank you.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

I suggest the following:

  1. Refrain from using coconut oil, as it may exacerbate dryness.

  2. I suggest choosing Episoft cleanser (Purified water, Cetearyl alcohol, 1,3 Butylene glycol, Maltooligosyl glucoside, Hydrogenated starch hydrolysate, Cetostearyl alcohol, Sodium lauroyl sarcosinate, Phenova, Acrylic acid, C10-30 Alkyl acrylate crosspolymer, Triethanolamine) or a soap-free cleanser for a two-month wash.

  3. Avoid rough handling and wear loose, comfortable, thin cotton undergarments.

  4. Steer clear of chronic friction, rubbing, or scratching.

  5. I suggest applying Candid cream (Clotrimazole 1 percent w/w) twice daily for 14 days; it should significantly clear the lesion within a few weeks.

  6. I suggest taking one tablet of Loratadine 10 mg after breakfast for 10 days (if experiencing itching or irritation).

  7. If burning during urination, I suggest taking syrup Cital 3 ml (Disodium hydrogen citrate - 1.37 gm per 5 ml) twice daily for seven days.

  8. For a three-day treatment, I suggest you consume one tablet of Fluconazole 150 mg after dinner once liver and renal functions are confirmed normal.

Consult your doctor and take the medicines.

I hope this helps.

Thank you.
