Hi doctor,

I am having red rashes on the glands of the penis. I used Candid Total and it disappeared completely. Again after seven days it again appeared and it seems red patches on right and left side of the penis. I continued using it and then got an advise form one of my friend for using Surfaz SN along with Doxt SL 100 which I am using it since last 15 days.

It was improving slowly. I was using very hot water for cleaning the penis but suddenly day before yesterday, the color of the penis became red and the patches have also become red. I am having pain on the glands of the penis and seeing some white discharge. I do not know why it has happened suddenly, due to the usage of hot water or for some other causes. I consulted a local doctor and he advised to take LNZ 600 one tablet twice daily for five days and T-Bact ointment twice daily for 10 days. Please advise.

You have erosions on the glans penis that is more consistent with candidal balanitis, other possibilities are chancroid, herpes. Please stop using Surfaz SN, it has steroid in it. Steroid helps to bring down the inflammation, itching will get better but it will not cure the fungal infection further makes the fungal infection worse. I believe that is what happened for you now.

Can you please answer these questions so that I can rule out other possible conditions and suggest you appropriate medications. When did it start? Do you have similar red rash on foreskin as well? Any history of unprotected sexual intercourse? Any history of white discharge in the female partner? Did you have fluid filled blisters when it started? Is this the first time you are experiencing this kind of lesion? Do you have any allergies to medications? Do you have any other medical conditions? Diabetes?

Do not use hot water for genitals. Always use lukewarm water.

It started three monthsback when I got cold and doctor prescribed me Levofloxacin 500 for five days. At that time I noticed rashes. I has a sexual intercourse that month but I used condom. I do not know about the female partners history. But after using lubricated condom, I found that my skin was itching. This is the first time I am experiencing this kind of lessions. I believe that I am not diabetic since I checked six months back and my HbA1c was 4.8.

Last month, I was having some pain in the ankle and my doctor suggested for Tendowell and these patches started appearing after having this medicine. I do not know whether for this it appeared or whether for some other infection it triggered.

It looks like candidal balanitis aggravated by application of topical steroid (Surfaz SN). Start using Clotrimazole 1 % cream, apply to affected area twice a day after cleaning the area with lukewarm water for 4 to 6 weeks. Take capsule Itraconazole 200 mg, one capsule in the morning and one capsule at night on day 1. It is single day treatment. Avoid unprotected sexual intercourse for 3 to 4 weeks.

Thanks for your advise. For your information I am a bachelor guy and I never indulge in any sexual intercourse and that too without condom. As informed earlier I had come across such incident once two months back and another two years back and all are protected intercourse. Anyways, I keep your advise in mind and will avoid any sort of sexual intercourse in future. Can you tell me why it happened? Why it agrevated since after using Surfaz it got cured 90 % but since day before yesterday after using extreme hot water it again appeared and aggrevated.

Candidal balanitis is the infection caused by candidal which is fungus. In males, galns and foreskin are more prone for fungal infection because the genital secretions (smegma) collects inside. When there is trauma (friction) to the area with minor cuts on genital mucosa, it gets infected. There are other factors like hygiene, excessive sweating can predispose for fungal infection. Diabetics have altered immunity making them more prone to fungal infection.

Surfaz Sn has steroid in it. Steroid helps to reduce the inflammation, hence it feels better initially when only the inflammation part (redness, itching) is gone, but fungus will still be there. Steroid makes the fungus grow further. After few days fungus will multiply causing more symptoms. Hot water dries the skin and mucosa, that will cause irritation to skin when there is infection. Lukewarm water should be used for taking bath and washing genitals.

Shall I stop taking LNZ 600 and shall I stop applying T-Bact oinment as advised by general physician?

If you have started the LNZ then complete the course. T-bact ointment is antibiotic, you can use it along with Clotrimazole.
