How to manage perianal abscess?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am a 48-year-old male. My height is 170 cm, and my weight is 89 kg. Around two and a half years ago, I started having the symptoms of abscess and pus around my anal area. So I went to the hospital, and the doctors said it was a fistula and needed surgery. For the past 11 months, I have had six fistula surgeries, and the pus is still coming. So I went to another doctor and did an MRI scan (I had previously not taken an MRI), and the results said it was a perianal abscess.

Hello,

Welcome to icliniq.com.

I can understand your concern. Can you send me a picture of the MRI (magnetic resonance imaging) report? Was any bacterial culture done? Please answer the above-asked questions so that I can guide you better about the medicines and investigations. I wish you good health. Thank you.

Patient's Query

Hello doctor,

Thank you for your response. The following is my MRI report. Can you suggest a treatment plan? I want to connect with a colorectal surgeon as well.

Hello,

Welcome back to icliniq.com.

I have gone through the reports you have attached (attachment removed to protect the patient’s identity). It would help if you had complete exploration in the form of Incision and drainage with regular dressing under general anesthesia. We need to send culture and give antibiotics according to sensitivity pattern for three weeks. Were any cultures performed previously?

Patient's Query

Hello doctor,

Thank you for your response. Should I be worried about the length of the perianal sinus track? It is about 15 cm long.

Hello,

Welcome back to icliniq.com.

Yes, it is a matter of concern. You should definitely get an exploratory surgery done under general anesthesia. I hope I have solved your query. I will be happy to help you further. I wish you good health. Thank you.

Patient's Query

Hello doctor,

Thank you for your response. How serious is my case, and how long will it take to heal? Are there any side effects after the surgery?

Hello,

Welcome back to icliniq.com.

It is not at all serious. However, it will take three to four weeks for complete healing. Side effects are not severe but delayed healing, scarring, and secondary infection are the possible side effects. I hope I have solved your query. I will be happy to help you further. I wish you good health. Thank you.

Patient's Query

Hello doctor,

Thank you for your response. Are there any abnormalities on the MRI report? Will the previous surgeries cause other problems as the previous doctors thought it was a fistula and did six surgeries on me, and the MRI report shows that it is not a fistula?

Hello,

Welcome back to icliniq.com.

I can understand your concern. However, this is a fistula complication due to incomplete drainage previously. Thank you.
