Hello doctor,

I have a hard swelling measuring 3/4th finger size from the anus on the right side. It hurts while sitting. It started developing five days back and has been worse since yesterday. The entire nearby area is sensitive and slightly painful. For your information, I underwent surgery for an anal fistula on the left side 52 years ago.

I have had type II diabetes for the past five years and BP for the past 12 years. I am 82 years old. Kindly suggest medicine, ointment, or injection for instant relief from pain. Please help.

I read your query and can understand your concern.

It looks like you have a right perianal abscess. It needs to be clinically examined by a surgeon and, if required, drained as early as possible.

You need to start antibiotics too, as you are diabetic, which increases the chance of the infection. My antibiotic preference would be Augmentin duo (Amoxicillin and Clavulanic acid) 1 gm twice daily for five days or Ciplox-TZ (Ciprofloxacin and Tinidazole) twice daily for five days. For pain, you can have tablet Ultracet (Acetaminophen) 1 three times a day or as needed. Consult your specialist doctor, discuss with him or her, and take medications with their consent.

Hi doctor,

Thank you for the reply.

I have taken the tablet Augmentin for five days. There is no pain now, and the swelling is slightly better. It has, however, not softened or ripened yet for draining. Please let me know whether I should continue the tablets for five more days.
