Hi doctor,

I am a 33-year-old male. I find it challenging to achieve a firm erection or maintain it long enough for satisfactory sexual intercourse. I have noticed a decrease in my sexual desire and overall interest in intimacy, which is affecting my relationship and self-esteem. I have attempted lifestyle changes, such as improving my diet and exercise routine, but I have not seen significant improvements. I am contacting you for your expertise in evaluating my situation and recommending appropriate steps to address my concerns.

I can understand your concern.

It can be due to:

  1. Stress due to work or family-related.
  2. Depression.
  3. Varicocele.
  4. Fear of failure.
  5. Low self-esteem (chronic masturbation).
  6. Homosexuality.
  7. Religious constraints.
  8. Smoking and alcohol.
  9. Long-term illness such as cardiac disease, thyroid disease, kidney disease, or any other disease with long-term medication.
  10. Negative attitude towards your partner.
  11. Hormonal issues such as thyroid issues and low testosterone.
  12. Long-term drug usage.
  13. Diabetes and hypertension.
  14. Need to check Doppler penis, USG (ultrasonography) pelvis, and scrotum, blood pressure, RBS (random blood sugar), free T3 (triiodothyronine), T4 (thyroxine), testosterone, and TSH (thyroid stimulating hormone).
  15. For a temporary solution, I suggest you take a tablet of Tadalafil 5 mg twice a day and a tablet of Dapoxetine 30 mg half an hour before sex for 30 days.
