Hello doctor,
This is the ninth day after my wisdom teeth extraction and I still have a painful swollen gland under my jaw. What is the cause? Kindly help.

I know it might be hard for you at this stage after a wisdom tooth surgery. Sometimes, the wisdom tooth surgery is done a little aggressively since the tooth may be lying deep into the bone and to remove it, a lot of work has to be done. Even some of the bone has to be removed to achieve this. But do not worry, this is a temporary phase and you have crossed the hard phase.
Hot and cold application in the beginning may help reduce the pain and swelling. You can still do cold application. You can buy a cold pack in the drug store, put it in the freezer, and then apply it to the area for at least 10 to 15 minutes. Also, take the painkiller that your doctor gave you. In case you do not have any prescription medicine left, try taking over-the-counter painkillers like Ibuprofen 400 mg at least three times a day. Use Listerine mouthwash (Eucalyptol, Menthol, Methyl salicylate, and Thymol) and clean the area with a soft brush.

Thank you for the reply.
But right now it has been a month since the operation and I still have a swollen lymph node next to the extraction area. Sometimes it is annoying, especially at night. Should I be worried? Kindly guide.

It needs to be investigated further with X-rays, clinical examination, etc. Is the stitch still there? Do you have Listerine zero (Sodium fluoride) mouthwash? Brush that area thoroughly and keep it clean by using a small head brush. And use over-the-counter painkillers for three to four days. If this does not help, then seek a doctor's advise soon.

Yes, I have Listerine zero mouthwash and I will try my best to keep it clean. The upper stitch is gone but the wound is till there. I will attach a picture. Please have a look.

I have seen the picture (attachment removed to protect the patient's identity). Yes, I can see one stitch there and sometimes stitches that persist longer than usual may be a source of infection. In your case, it may not be the cause but it is better to remove the stitch. Use Listerine zero (Sodium fluoride) 15 ml mouthwash for 30 seconds. If you are using it after toothbrushing, then use it at least 30 minutes after brushing. You can take painkillers for pain. If not allergic, take Advil (Ibuprofen) 400 mg twice a day. Once you are fine, visit a dentist or hygienist to remove the stitch.
