Patient's Query

Hello doctor,

My father suffered a stroke. The CT scan results showed that this was his second stroke. An injection was administered to break up the clots. It is been almost two months, and he is still very restless and constantly wants to go out. His speech is also not proper, and he has run away from the house a couple of times. We have to keep all doors locked at all times. He is prescribed Quetiapine 50 in the morning and at night. Kindly advise us

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern. It may be possible that this is related to the stroke. It is referred to as post-stroke depression or psychosis. You can consider starting an antipsychotic medication like Olanzapine at a dosage of 2.5 to 5 mg. Quetiapine can also be helpful, but at a dosage of 50 mg, it can be sedative, so you may want to begin with 25 mg, taking half in the morning and the full dose at night. Please wait for two to three days to see the results.

Thank you.

Patient's Query

Thank you doctor for the reply,

I am attaching his reports below for better understanding. Also, please note that he is very agitated at times. As I mentioned earlier, we are keeping all doors locked and staying with him inside our house to prevent him from going anywhere, which has led to him yelling loudly. In response, the doctor suggested giving Quetiapine 50 mg in the morning and evening. Previously, he was prescribed Quetiapine 100 mg at night. The situation has not significantly changed; the only difference is that during the period when he is sedated, he is less agitated.

Hello,

Welcome back to icliniq.com.

I understand your concern. You should begin with Olanzapine 5 mg in the morning and afternoon and Quetiapine 50 mg at night. This kind of agitation may worsen if you restrict him. It is essential to address the underlying problem for a more permanent solution. Additionally, consider starting medications for neuronal degeneration, such as Donepezil and Memantine. Try to ensure he gets exposure to sunlight, and avoid turning off all lights at night.

Thank you.
