Hi doctor,

The first day of my last period was 29 days. I have been sexually active twice this month with preventive measures one night and without the next time, but I was not in ovulation period. And now I was expecting my cycle this past Saturday, but I am four days in, and I still did not get the periods. I took a pregnancy test yesterday morning with the first urine in the morning, and it came back negative. I have no pregnancy symptoms except sore nipples. And I have been stressing about it, not sure if stressing over is delaying my cycle or if I am pregnant? I need answers as soon as possible and if there are any alternatives to stop what is happening.

I have also gone through the details presented by you. I will request if you can share the details of intercourse dates to assess the pregnancy chances based upon the last menstrual period and ovulation pattern. I agree with you that stress also leads to delay in periods, and presently periods of almost 25% of females are delayed worldwide due to stress, including COVID stress. However, if your period is delayed by four days, then presently, there is no need to panic, and we can wait for another three to four days. Blood hCG (Human chorionic gonadotropin) test gives accurate results for pregnancy after 11 to 14 days. But urine pregnancy test is slightly inaccurate in early pregnancy. We need to be somewhat careful also while using urine pregnancy tests as it is time-sensitive. You have done it with the first urine of the day that is great, and keep in mind the timing to see the test. Do not drink too much fluid before the test. The first interaction was before 18 days, and I used preservation. The second time was before 13 days, and I was not ovulating, so I did not think I needed to use one. Tomorrow will be the fifth day, and it is also almost the end of the month. My cycle is not usually regular, but I also never would go a month without getting my cycle.

Thank you for the additional information. You have said that your cycles are not usually regular and your last period was before 29 days. So if it is a 28-day cycle for you, then you are not delayed. So I think there is no need to panic as classically you have not missed period. How many days period length you had in your last period? Also, I recommend not trusting the ovulation pattern unless you have fixed cycles. The ovulation may get slightly early or slightly delayed as you have said that periods are not regular. So, if you have thought that you were not ovulating may not be entirely correct. Even if, in the worst case, we assume that you were ovulating before 13 days and you conceived still, it is only 14 days and too early to detect such early pregnancy other than a blood hCG test. Kindly share your date of the last day of the period and your menstrual pattern of the previous two months to assess your likely ovulation dates and when your period was due this month, and the probability of getting pregnant.
