I am 29 years old. I had two miscarriages. I am pregnant again, but I am bleeding right now. Last week during the ultrasound, heartbeat was not detected. Is it a miscarriage?

I understand your concern.

I am so sorry for the outcomes of your previous pregnancies—a heartbeat though a vital sign of a viable pregnancy. An ultrasound scan can detect it only from five to six weeks of gestational age. Bleeding from the vagina is significant, but pregnancies can still be maintained even when there is a vaginal bleed as long as the cervix is closed. Progestogens such as a Norethisterone weekly intramuscular injection can sometimes avoid early pregnancy loss. You may need a repeat pelvic ultrasound scan to determine if the fetus and other products of conception are still present in the uterus and if your cervix is closed. Also you may need to check your packed cell volume to determine that the level of blood loss is not too significant. Consult a specialist doctor and take medications with their consent.
