Hello doctor,

I and my partner had protected dry sex on the very first day after her periods ended. Everything seemed fine. She started her periods normally on the exact dates she was supposed to, which was 25 days later. But the blood flow was heavy with clots and darker color and consistency. The period lasted for five to six days. Her periods have been regular in the past. She is currently not taking any medications, and no tests have been done yet. However this month she has not yet started her periods and it has been five days since her expected start date. Is there any possibility that she might be pregnant?

I read your query and can understand your concern.

I do not see any reason for you to worry. If you are confident that you had absolute protection, and she was just after periods, then there is no chance of pregnancy.

For any woman, ovulation usually occurs around the 12th to 16th day of her period. The life of an egg is 24 hours, and that of sperm is 72 hours. So there is no chance for the egg to be available on the day periods ended and sperm will not be alive till the next ovulation. Unless you had a second sexual exposure around ovulation.

Pregnancy is not the only reason for missed periods. Other factors to consider include:

1. Thyroid hormone and prolactin hormone abnormality can be a cause. I suggest having them tested.

2. Any physical or mental stress- happiness, sorrow, or anxiety.

3. Any sudden fall or increase in weight.

4. Any emergency contraception or any other hormones taken.

5. Significant infection in the vagina- usually with the white or yellowish discharge, with or without itching or foul smell, feverish feel during periods, thicker and dark red or brownish blood during periods, pain during periods and sex (especially towards the end) needs evaluation by a gynecologist and treatment for both antibiotics and antifungals.

To start with, have a urine pregnancy test, with an early morning urine sample.

Once confirmed negative, have any of the oral combined contraceptive pills (Birth control pills- packet of 21 pills) one tablet twice a day for 10 days, (throw away the 21st pill and inactive or dummy pills if any), no matter what the cause is (except pregnancy), she will have periods within seven to 10 days.

After the bleeding is over, she can have a scan to evaluate her uterus, to rule out any local problems, like polyps or fibroids (as you said bleeding with clots).

Her weight is in the lower limit of normal, so this poor nutrition could be the reason for no periods and leading to low immunity and vaginal infections. So I would advise her to have a lot of fruits and vegetables, milk, egg whites, sprouts, and chicken flesh only. Your expected weight is 161.55 to 174.16 pounds, BMI (body mass index) is 24 kg/m2.

Do load with unhealthy junk food, It will only spoil your health. High thyroid also could reduce our weight.
