What steps can help manage premature ejaculation?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am experiencing a seminal leak and pain in my left testis when I get aroused, even without engaging in sexual activity. The situation worsens when I am aroused after exercising. This seminal discharge is causing me anxiety and pain. I have been dealing with this problem for the past 10 years.

Please help.

Hello,

Welcome to icliniq.com.

I have read and understood your problem.

It may be due to an infection of the epididymis or prostate. You should get an ultrasound of the scrotum and prostate, as well as a bacterial culture and drug sensitivity test of the prostatic massage fluid. A course of suitable antibiotics may help after these tests.

Regards.

Patient's Query

Hello doctor,

Thank you for your response.

Could this condition also be a cause of premature ejaculation? If so, what can help?

Hello,

Welcome back to icliniq.com.

Yes, it is similar to premature ejaculation (PE). It is usually not associated with any disease, but it may be due to anxiety. However, sometimes there could be an issue with the prostate gland or thyroid hormone. You can learn to control PE by doing the following exercises:

  1. Pelvic floor exercises: At the end of urination, pull your pelvic muscles for a few seconds and then release. Repeat this several times daily.
  2. Stop-start technique: When you feel that you are about to ejaculate, stop the sexual stimulation and divert your mind. Once the penis relaxes, start the stimulation again and repeat the process.

If you continue these exercises, over a few months, you will gradually increase your ejaculation time. Most often, PE is a chronic issue with no permanent cure, so you may need to take medications as required.

I hope this helps you.

Thank you.
