Hello doctor,

I am a 36-year-old individual. During intercourse, my penis gets an erection, but I ejaculate in less than a minute. It is becoming difficult. I am losing confidence. Glans penis (head) is completely covered by foreskin and cannot be seen when tried to retract. It is painful and not possible to retract but when not erect, the foreskin can be retracted without pain and be cleaned. The main problem is premature ejaculation. I also have diabetes.

Premature ejaculation is very common and is usually not associated with any disease but may be due to anxiety. However, it is sometimes associated with infection of male accessory glands, which is treatable. You can learn to control premature ejaculation by doing this exercise. Pull your muscles that you pull at the end of urination for a few seconds and then release. Repeat this several times daily. Another is the stop-start technique. When you feel that you are going to ejaculate, stop the sexual stimulation and divert your mind, the penis will relax, after some time, start the stimulation again and repeat the procedure. Over a few months, you will gradually increase your ejaculation time. Also, there are medicines to control it but there is no permanent cure. In your case, you should strictly control diabetes and reduce weight by doing slow walking daily for 15 minutes. Your foreskin may be tight due to some infection which is common in obese diabetics. If you get too much pain, then circumcision, which is a small operation can help.
