Hi doctor,

I am a 32-year-old married women. I am having a period of 146 days that continues with a low and sometimes medium flow. I have consulted a doctor here but he found everything so normal in USG and I did LH, FSH, TSH, T3, T4, prolactin, blood sugar, and beta hCG also. All test results are absolutely normal. The doctor said let it be everything is normal, he did not even get done my hemoglobin and he did not even give me Trenaxa or any medicine. I am living with my husband without having sex. I tried a lot to convince him to do sex. He tried and failed and he found himself normal with not having sex since two years of marriage not even once. Maybe my hormones fluctuated and that is why I am having irregular periods. And I need sex as I am married but I am not getting it. Can it be the reason for continued bleeding? The doctor did not find any cysts also in my ovary.

Please suggest what to do.

Do you have recent hemoglobin and thyroid function tests reports?

Go for one transvaginal scan, and complete blood count. It would be better if you would have an outpatient hysteroscopic examination.

Start iron supplements immediately and eat food filled with iron like carrots and apples. Take Transenexamic acid one gram eight hourly for one day and then reduce it to 500 milligrams for two to five days as per required. Take this with Ponstan (Mefenamic acid) 500 milligrams eight hourly. Start tablet Norethisterone 10 milligrams twice daily from the fourth day of periods and continue till day 26 and stop. Observe if you get some relief. Again repeat this from day four to day 26 of periods. Continue for three cycles to regulate cycles and to have less bleeding. Collect laboratory results and discuss them here or local gynecology doctor. During bleeding, you can take Tranexamic acid and Ponstan from day two to five of bleeding. Consult with your doctor and take medicines with the consent of the doctor.
