I have gone through the history that you have provided along with the images (attachments are hidden to protect the patient's identity) uploaded, and I do understand your concern.
As for the information provided, the concern that you are going through is palmoplantar psoriasis, which is a form of inflammatory condition that happens on the skin. There are two types of psoriasis, type 1 and 2. The first type of psoriasis usually occurs in cases of any family history of the same skin condition and gets genetically transmitted whereas the second type, which is quite common, usually occurs at a later stage in life, most probably after 30s, and usually occurs due to multiple reasons including stress, lifestyle, dry skin, dietary changes, vitamin deficiencies especially vitamin D3, and any stressful conditions in the body. Although psoriasis has a characteristic of being on and off and may recur in the future most often for most individuals with few exceptions, the current issue can be managed with medications, which will help in reducing the symptoms as well as the appearance of the rashes. Following are my suggestions:
1. Lozivate-MF ointment (Clobetasol and Fusidic acid). Apply a thin layer on the affected area at night for three weeks. Stop after three weeks and may take a gap of three weeks if needed each time for two weeks again.
2. Logifeel cream (Glycolic acid and urea). Apply in the morning and afternoon as a moisturizer in the affected area daily.
3. Capsule Seacod (Omega 3 fatty acid and cod liver oil). Consume after dinner daily for two months.
4. Capsule Evion (vitamin E) 400 mg consumed after dinner for two months.
5. Capsule Lumia (Cholecalciferol) 60000 international units once a week after dinner for six weeks.
6. Lukewarm salt water and 1 tablespoon of coconut oil mixture can be used for soaking hands and feet for 10 minutes daily in the morning.
7. Oilatum emollient (liquid paraffin). Use three caps full of the product using the bottle cap, mix in normal temperature water, and soak feet and hands for ten minutes in the evening daily.
8. Oilatum soap (medicated soap for dry and sensitive skin) may be used for bathing.
9. Avoid food that increases body temperature, like hot and spicy food, red meat, and alcohol, and use food that is calming to the body, like fruits, vegetables, and curd, and consume at least 0.79 liquid gallons of water per day.
10. Stress management is also advised.
The idea is to keep the affected area soft and avoid scratching to prevent infections.
