Hi doctor,
I have been experiencing severe itching near the pubic area, and the skin has turned dark. Previously, I had an issue with pubic lice, which was treated successfully with Permethrin lotion. I am currently using a combination of Ciclopirox and Fluocinolone acetonide cream, but it is not effective.
Could you please suggest some medications for this condition?

You are experiencing tinea incognito (a fungal infection masked by steroid use). Steroids are contraindicated for fungal infections, and using a steroid in this case may have worsened the condition. However, I recommend the following medications. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.
- Capsule Itraconazole 100 mg twice daily for 10 days.
- Tablet Levocetirizine one tablet twice daily.
- Apply Terbinafine cream twice daily.

After 10 days, the infection has subsided, but the itching and darkened skin persist. The itching is sometimes very severe.

The darkening of the skin is likely to take a month to subside, as it is post-inflammatory hyperpigmentation caused by an increase in melanocytes. In the meantime, you can apply a depigmenting gel twice daily. I recommend using a combination of Montelukast and Levocetirizine as long as the itching persists and keep the area dry by applying Clotrimazole powder. Avoid wearing tight undergarments to prevent recurrence, as the condition is prone to coming back whenever the fungus finds a suitable environment to grow.
