Hello doctor,

Small lumps and rashes have appeared on my penis on and off over the last four months. I went to a dermatologist, and he or she assured me it was probably only an itch and nothing to worry about. Having said that, the symptoms are still present. Non-steroidal creams, antifungal creams with oral medication, and steroid creams are among the remedies I have tried. At times, the symptoms subside, but they reappear at regular intervals.

I cannot give a definite diagnosis just from the image, but it looks like there is some mild dryness and irritation on the head of the penis, along with a few small bumps. I do not see any clear ulcers, open sores, pus, or crusting.

These symptoms have recurred over the past four months and have not resolved despite various treatments. A comprehensive medical evaluation is needed rather than assuming a simple irritation.

Possible causes include:

  • Irritant or friction dermatitis, which may result from exposure to soaps, perspiration, masturbation, condoms, or lubricants

  • Chronic balanitis, defined as persistent inflammation of the glans peniseated irritation caused by yeast

  • Repeated irritation caused by yeast

  • Eczema or a contact allergy affecting the skin on the penis. For the past four months, I’ve had rashes and small bumps that keep returning. I saw a dermatologist who thought it was a minor irritation and nothing serious, but the symptoms haven’t fully gone away. I’ve tried treatments like steroid creams, antifungal creams with oral medicine, and non-steroidal creams. Sometimes the symptoms improve, but they keep coming back.

Based on the image, there are no clear signs of a serious infection. However, a physical examination and further testing may be needed to confirm this.

Repeated or prolonged use of steroid creams on genital skin can cause additional problems. Although steroids may temporarily reduce inflammation, inappropriate or extended use can thin the skin, worsen irritation, change skin color, or allow fungal infections to persist. These issues are especially common with combination creams for “fungal infections” that contain steroids.

Since your symptoms have continued for several months despite treatment, I do not recommend using additional creams without a confirmed diagnosis. A targeted medical evaluation is the best next step.
